This is a FCS-only head mold. The Full Choice Handbook: 2016 by Volks shows the head mold with 18mm eyes. It was released in Japan in September 2016 as a Tenshi no Sato exclusive. On September 16, 2023 it was released to Tenshi no Mado and Sato locations. It also had a limited release at Tenshi no Mado from October 7th through December 13th, 2022.
